Facility Evaluation Report

On 4/15/26 at 12:00pm Licensing Program Analyst (LPA) J. Leffall conducted a case management regarding an Incident report received by the Department. LPA met with Licensee (L1) Ron Camilo and stated purpose of visit.

The department conducted interviews and reviewed records. Based on an incident report received by CCLD on 3/30/26 the Staff (S1) yelled over resident while on duty on 10/29/25. Per report the staff has received disciplinary action, where S1’s employment was terminated.

The following deficiencies is being cited on the attached LIC-809D in accordance with California Code of Regulations, Title 22, Division 6.
