Facility Evaluation Report

On 7/3/25 Licensing Program Analyst (LPA) J. Leffall arrived at facility to conduct an initial complaint and delivered findings. LPA introduced self and was granted entry. 3 staff were present during visit. LPA was met by Staff (S1) Gloria Zuniga. Staff (S2) Shona Espinosa and Erwin Villaries were also present upon LPA's arrival. Licensee (L1) Ron Camilo arrived shortly after LPA's arrival. After observation and interview, LPA was informed both staff are new to facility. LPA verified both new staff to be associated to facility. Based on record review, 1 out of 3 staff are not associated with facility.

The following deficiency is being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6. Civil penalty issued for staff not being fingerprinted and/or associated to facility.
