Facility Evaluation Report

On 5/28/26, Licensing Program Analyst (LPA) J. Leffall conducted a case management visit to follow up and confirm details of an incident report that was received by the Department. The incident occurred in which R1’s medication of Famotidine officially ran out on 5/18/26. To date, the medication has not refilled. LPA met with Staff (S1) Irish Lobos. Administrator will discuss with LPA plan that is in place that will prevent residents from running out of medications in the future.

The following deficiencies are being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6.
