Facility Evaluation Report
Licensing Program Analyst (LPA) Daiquiri Boyd arrived unannounced to conduct an annual visit. LPA introduced self, stated the purpose of the visit, and completed this annual visit with Senior Director, Bridgette Franco (S1). There were approximately 100 clients at the facility during the inspection.
Senior Director stated that they are approximately a month away from moving into their new facility.
LPA toured the building and observed fire exit of the facility to be clear and from obstruction. LPA toured the facility inside and out. The facility was observed to be at a comfortable temperature, 69/70 degrees F, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. Water temperature was checked in the kitchen and measured at 115.6 degrees F. Last disaster drill was completed on 03/04/2025. LPA reviewed a sample of five staff and client's records. Fire Department tested their fire alarms on 1/11/2025, pending the clearance inspection letter. Fire extinguishers were charged and was serviced on 10/08/2024.
LPA observed a 30-day supply of PPE. Medications are currently not stored or administered at this facility. The facility has one client that keeps an inhaler on site in case of emergency, but the client hasn't used it in over a year. Cleaning supplies and chemicals stored and locked in a closet on the main floor.
There was outdoor seating for the residents. Outdoor area was clean and free of obstruction.
