Facility Evaluation Report
Licensing Program Analyst (LPA) M. Flores arrived unannounced to conduct an annual visit. LPA introduced self, stated the purpose of the visit, and completed this annual visit with Senior Director, Bridgette Franco (S1). Licensee, Jami De La Cerda (S2) arrived at the facility to complete this annual visit. There were approximately 100 clients at the facility during the inspection.
LPA toured the building and observed fire exit of the facility to be clear and from obstruction. LPA toured the facility inside and out. The facility was observed to be at a comfortable temperature, 69/70 degrees F, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. Water temperature was checked in two of the bathrooms and measured at 117- and 116.2-degrees F. Last disaster drill was completed on 11/03/2023. LPA reviewed a sample of five staff and client's records. Fire Department tested their fire alarms on 12/08/2022. Fire extinguishers were charged and was serviced on 10/20/2023.
LPA observed a 30-day supply of PPE. Medications are currently not administered at this facility. Cleaning supplies and chemicals stored and locked next to the elevator on the first floor.
Deficiencies are being cited based on LPA observations and record review in accordance with the California Code of Regulations, Title 22, see LIC809D. S3 and S4 are not fingerprint cleared to work in the facility.
There was outdoor seating for the residents. Outdoor area was clean and free of obstruction.
Exit interview was conducted and a copy of this report LIC809 was provided to Licensee, Jami De La Cerda. The following documents are requested and submitted to Fresno CCL by Administrator. Forms requested: LIC308, LIC 309, LIC 400, LIC 402, LIC 500, LIC 610D, LIC 9282, and training logs for S2.
