Facility Evaluation Report
On 6/19/25 Licensing Program Analyst J. Leffall arrived unannounced for a case management visit regarding a Default Decision and Order for Staff 1 (S1) LPA met with Staff Olga Lopez. Administrator (A1) Roxanna Baghetti was called and arrived shortly after LPA’s arrival. Upon arrival A1 verified Rina Tafolla is not employed or associated to facility. LPA conducted visit to verify an individual that no longer has a criminal record clearance is working at the facility.
LPA was informed by Administrator that S1 is not employed at the facility and no longer associated with the facility.
Exit interview was conducted. A copy of this report was provided to Administrator, whose signature confirms receipt of this report.
