The inspector’s account

LPA Luisa Fontanilla conducted a case management related to complaint 15-AS-20250320143652. LPA met with Administrator Jonnasburg Carbonel.

During the course of investigation, LPA reviewed records and interviewed staff.

C1’s IPP indicates that home staff uses a hoyer lift to assist with transfers.

Based on observations made by LPA during the visit and based on interviews conducted, staff do not use the hoyer lift in transferring C1. Instead, two staff would transfer C1. One staff behind C1 holding both underarms and one staff holding C1’s legs. Staff state C1 refuses to use the hoyer lift.

When LPA touched C1’s feet and felt they were really cold, LPA asked staff if C1’s doctor has been notified. C1’s primary doctor has not been notified about C1’s cold feet.

Deficiencies are cited per Title 22 California Code of Regulations (refer to Lic 809D).

Exit interview was conducted and Appeal Rights was provided.
