On this day while at the facility for the annual inspection, LPA inquired from the home health nurse present which client the nurse is visiting. The nurse said that she is visiting one client for wound treatment.
LPA obtained information from the nurse that Client 1 (C1) has one stage 1 pressure injury in the buttock and three (3) unstageable pressure injuries. LPA obtained a note from the nurse.
The facility does not have an approved exception to retain client with prohibited condition.
Civil penalty determination related to serious bodily injury is pending.
Exit interview was conducted with the Administrator.
