Facility Evaluation Report
At around 1:50 PM, Licensing Program Analyst (LPA) Luisa Fontanilla arrived announced to conduct a case management - Licensee initiated visit and met with Administrator Debbie Rieza.
On 10/17/2025, the Administrator reached out to LPA to notify about Client 1 (C1) decline in health condition and possible change in ambulatory status. On 10/21/2025, LPA interviewed day program staff who assists C1 with incontinence care. The DSP confirmed with LPA that C1 is a two-person assist and is unable to turn from side to side independently.
On 10/24/2025, LPA observed C1 unable to turn from side to side independently. Staff needed to turn C1 from side to side to provide incontinence care.
Health and Safety Code 1568.0832 defines bedridden as " requiring assistance in turning and repositioning in bed.... The facility has an approved fire clearance for four (4) non ambulatory clients only.
The Administrator will: notify the Union City Fire Department within 24 hours about C1's change in ambulatory status and will notify CCL.
