Facility Evaluation Report

On this day at around 12 pm, LPA K. Nguyen arrived unannounced to conduct annual required inspection and met with staff Josefina Dauz. LPA explained to Dauz the purpose of the visit. The Administrator Judy Salazar arrived at the facility at around 12:10 pm.

During the visit, LPA inspected the facility inside and out including but not limited to client rooms, bathrooms, kitchen, dining, garage and backyard. The facility has an approved fire clearance for 4 non ambulatory clients. The facility is a Level 4F home vendorized by the Regional Center of the East Bay (RCEB). LPA observed no client at the facility upon arrival. Total Five clients four clients were in their respective day programs, and one is in skill nursing facility.

Hot water measured at 111 Fahrenheit. There was sufficient supply of perishable and non perishable foods. Sufficient supply of hygiene products, warm blankets, towels and sheets in good repair available for client use were observed. Fire Drill last conducted on 12/25/25. Fire extinguisher last checked on 1/16/26. Surety Bond effective 3/3/23 to 3/3/27.
