Facility Evaluation Report

While at the facility conducting annual inspection, LPA observed Client 6 (C6) ambulating with one staff. LPA noticed C6 walking unsteadily, leaning on the staff with eyes closed. LPA asked staff if C6 is able to ambulate independently. C6 room is on the 2nd level of the facility. C6 Physician's Report indicates C6 is ambulatory.

LPA interviewed 3 staff on site to find out if C6 is able to ambulate independently. Staff interviewed state that staff have been assisting C6 with ambulation recently due to C6

The Administrator will notify the local fire department within 24 hours and let LPA know. Also, C6 will be brought to the doctor for reassessment.
