Facility Evaluation Report
On 1/16/2026 at 9:30 am, Licensing Program Analyst (LPA) Y. Brown arrived at the facility unannounced to conduct an annual required inspection. LPA met with Co-Administrator Charisma Aguilar and explained the purpose of visit. The facility has an approved fire clearance for four (4) ambulatory clients.
LPA toured facility with Co-Administrator including but not limited to 4 bedrooms which 3 bedrooms will be occupied for clients and one staff room, 1 office/receiving area, 2 bathrooms, kitchen, common areas and backyard. Bedrooms and living rooms were equipped with the proper furniture. Bathrooms were equipped with grab bars and non-skid mats. Linens and hygiene supplies were observed inside a cabinet. There is sufficient lighting throughout facility. Room temperature was maintained at 68 degrees F and hot water temperature was maintained at 108.8 degrees F. First-aid kit was observed to be complete. Smoke detectors and carbon monoxide were operational. Fire extinguisher was last serviced on 1/16/2025. LPA observed and reviewed the Emergency Disaster plan was last reviewed 1/16/2025. The facility has a surety bond in the amount of $2,000 which is sufficient to cover amount of money being handled at one time.
LPA observed that there are currently no clients residing at the facility at this time. LPA observed a minimum of one week supply of non-perishable and two (2) day of perishable foods. LPA observed a cabinet for medication and a drawer for sharps were locked and inaccessible to clients.
