Facility Evaluation Report
On this day, July 29, 2026, at 4:35 p.m., Licensing Program Analyst (LPA) Delmundo arrived unannounced to conduct an annual required inspection. LPA met with Aleta Mendiola, staff, and informed the reason for visit. LPA called and spoke over the phone with Brenda Huynh, administrator, who stated she cannot come to the facility and authorized Aleta Mendiola to sign and receive this report. LPA also met with other staff, John Mendiola.
LPA toured the facility inside and out with Aleta Mendiola. LPA inspected the kitchen, dining area, living room, bedrooms, bathrooms, garage, front, side and backyard. Facility has sufficient perishable and non-perishable foods.
Fire extinguisher was observed fully charge with tag showed serviced June 1, 2026. Facility has carbon monoxide and smoke detectors that were tested and observed in operating condition on this day. Hot water temperature in the ensuite bathroom was tested. Facility conducts disaster drills almost monthly and records showed earthquake and fire drills last conducted May 18, 2026 and July 8, 2026 respectively. Central storages for medications and cleaning supplies were observed locked.
at 4:54 pm, broken faucet knob in the ensuite bathroom.
at 4:55 pm, moldy shower area with cracked tiles in the ensuite bathroom.
at 4:58 pm, hot water at 101.4 degrees Fahrenheit.
