Facility Evaluation Report
On this day at around 10:05 am, Licensing Program Analyst (LPA) Luisa Fontanilla arrived at the facility unannounced to conduct an annual required inspection. LPA was met by the Administrator, Brenda Huynh. LPA explained to Huynh the purpose of the visit.
The facility is a Level 4C home vendorized by the Regional Center of the East Bay (RCEB). It has an approved fire clearance for 6 ambulatory clients.
Upon arrival, LPA observed one client eating in the dining area. The Administrator states all the other clients are in their respective day programs.
LPA inspected the facility inside and out including but not limited to 4 client bedrooms, bathroom, kitchen, dining area, garage and backyard. There was no body of water observed. Hallways and passageways were observed free from obstruction. Hot water measured at 105.4 degrees Fahrenheit.
Fire extinguisher was observed full and last inspected on 6/4/2024. Carbon monoxide and smoke detectors were tested and observed functional. There was sufficient supply of perishable and non perishable foods observed. Medications were observed locked in a cabinet in the hallway. There was sufficient supply of towels, sheets and warm blankets observed.
At around 10:40 am, LPA reviewed P&I money and log. The facility has sufficient amount of surety bond to cover cash being handled at one time.
At around 10:55 am, LPA reviewed 5 client files and 3 staff files.
