Facility Evaluation Report
On 07/17/2025 at 4:30 PM, Licensing Program Analysts (LPAs) L. Alexander and K. Nguyen arrived unannounced to conduct a Plan of Correction (POC) visit. LPAs met with caregivers, Renato & Zenaida Tisico and explained the purpose of the visit. Zenaida phoned Administrator, Victoria Puruganan to inform. Victoria Puruganan arrived approximately 1 hour later.
On 07/16/2025, LPAs conducted an Annual Inspection visit in which deficiencies were cited. The POC due date was 07/17/2025. Administrator failed to submit the POC by the due date and this is why LPAs came to make a POC visit.
CCR 87202(a)(2) Fire Clearance: for having Resident, R2, in a bedroom without fire clearance for bedridden.
Administrator will send LIC 200 and facility sketch to CCLD.
Immediate Civil Penalties in the total amount of $500.00 is assessed today for failure to meet POC date for deficiency
The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22 and/or Health and Safety Code Failure to correct deficiencies by POC date may result in additional Civil Penalties.
