Facility Evaluation Report

On 01/16/2026 at 1:45 PM, Licensing Program Analyst (LPA) P. Manalo arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Program Director, Gina Rivera, and explained the purpose of the visit. Program Director had to leave for an appointment and gave verbal authorization for Ferdinand Ramirez to sign the report.

Program Director, Gina Rivera, stated that the facility is undergoing construction and the Adult Day Program (ADP) participants have been relocated to Green Oak Developmental Center II and other participants have been attending the ADP via Zoom meetings. Program Director sent an email to CCLD on 12/30/2025 that the construction will begin on January 12, 2026, and will take approximately a month to complete. LPA informed staff to notify CCLD once construction is complete.

During the visit, LPA conducted a toured around the facility to verify the construction being done and that no clients are present in the facility. LPA obtained a copy of the participant roster of the clients that was relocated to Green Oak Developmental Center II.
