Licensing Program Analyst (LPA) Luisa Fontanilla arrived unannounced to conduct a case management visit and met with Interim Administrator Remedios Sullera. LPA explained to Sullera the purpose of the visit.
LPA conducted this visit as follow up of the visit conducted by DDS Nurse Consultant on April 2, 2025.
Deficiency is cited per Title 22 California Code of Regulations.
Exit interview was conducted with Sullera and Appeal Rights was provided.
