Facility Evaluation Report

At around 2:30 pm, LPA Luisa Fontanilla arrived unannounced to conduct a case management visit as follow up on the DDS Semi Annual Review conducted on October 7, 2025. LPA explained to Sullera the purpose if the visit.

During the semi annual review, the following were observed:

screen windows in Room #2 were observed to have significant gaps which potentially can allow insects to enter the rooms

medication cart was observed with cracks which can potentially compromise the safety of the medications in the cart

Client 1 has ordered restrictions but facility menu does not indicate C1's ordered restrictions

During the visit, Sullera provided the following updates:

Sullera followed up with the vendor and was informed that the contractor will come back tomorrow to measure screen window and get it ordered

Sullera states that the dietician has visited on 10/11/25 and provided menus for C1 restricted diet
