Facility Evaluation Report

At around 1:30 pm, LPA Luisa Fontanilla arrived unannounced to conduct a health and safety visit in connection with complaint 15-AS-20251008152859. LPA met with DSP Carlo Magno Olano. The Administrator Betchie Terrado arrived at approximately 2pm . LPA explained to Terrado the purpose of the visit.

During the visit, LPA inspected the facility and observed one client and one staff on shift. The two other clients arrived from the day program at around 1:45 pm.

LPA also observed a lady coming out of one of the rooms. LPA asked if she is an employee, she responded "No." LPA will have to come back to issue citation, if needed.

Deficiencies were cited per Title 22 California Code of Regulations (see Lic 809D).

Exit interview was conducted with Terrado and Appeal Rights was provided.
