On 12/4/2025 at 3:15PM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conduct a POC (proof of correction) inspection. LPA met with Administrator, Alejandria Bautista informed her the reason for the visit.
The following deficiency was cleared by visit :
- 87458(c)(1)(A); LPA observed R6 had TB test results on file during inspection.
No deficiencies are being cited on this date.
