Facility Evaluation Report

On this day at around 11:06 am, LPA Luisa Fontanilla arrived unannounced to conduct a case management - legal/non-compliance visit. LPA rang the door bell a few times but no one answered the door. LPA contacted the Administrator and was told that all staff are out. While typing up the report, LPA observed staff Joemar Lagunte arriving at around 11:18am. LPA spoke with Lagunte and explained the purpose of the visit. The Administrator was informed over the phone and she authorized Lagunte to sign the report.

During the visit, LPA interviewed Lagunte and obtained updated Lic 500. Based on review of Lic 500 and interview with Lagunte, the facility has two night shift staff - one works M-F and the other Sat-Sun. Lagunte stated no client has skin issues. One client is currently on a summer camp. The other two clients are in their respective day programs.

No deficiency is noted for today's visit.
