Facility Evaluation Report

On 8/13.2025 at 1:10 pm, Licensing Program Analysts (LPAs) K. Nguyen and P. Manalo conducted a case management due to the complaint (15-AS-20250805122017). LPAs met with Alejandria Bautista, Assistant Administrator, and informed the reason for the visit.

LPAs provided educational information to the Administrator regarding compliance with California Code of Regulations (CCR), Title 22, Division 6, Section 87608(a), which states: “Residents shall be free from any physical restraint, unless the restraint has been authorized by a physician for a specified and limited period of time, and only if absolutely necessary to protect the resident or others from injury.”

The administrator will provide in-service training to care staff under the regulation of Title 22, and submit to CCLD by 8/20/25.

An exit interview was conducted, and a copy of this report was provided.
