COMPLAINT CONTROL NUMBER: 15-AS-20220506104139

On 5/6/22 at 1:35 p.m. Licensing Porgram Analyst (LPA) Greg Clark arrived at the facilty to investigate the above allegation. LPA was told by care staff that R1 does not live at this facility.

This agency has investigated the complaint alleging illegal eviction We have found that the complaint was UNFOUNDED, meaning that the allegation was false, could not have happened and/or is without a reasonable basis.
