Facility Evaluation Report
On 12/18/2025 at 2:45 AM, Licensing Program Analyst (LPA) Y. Brown arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Administrator, Pamela Brown and explained the purpose of the visit. The administrator currently holds a certificate (# 7004560735 ) that expires on 6/9/2026. The facility’s fire clearance was approved for a capacity of six (6) ambulatory only.
LPA toured the facility inside and out including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 4 bedrooms and 3 bathrooms. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 73 degree Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. All toilets, hand washing and bathing are safe, sanitary and in operating condition. There is sufficient supply of hygiene products available for clients. There is a minimum of one week supply of nonperishable and 2-day perishables food supply. Centrally stored medication and sharps were locked and inaccessible to residents.
Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced on 11/21/2025. Emergency Disaster Plan was last reviewed on 1/10/2025. First aid kit was observed to be complete. Fire drill was last conducted on 11/9/2025
