Facility Evaluation Report

On 2/10/2026 at 9:30 am, Licensing Program Analyst (LPA) Y.Brown conducted an unannounced annual required inspection. LPA met with Program Director Marisol Salcedo and explained the purpose of the visit.

LPA toured the facility including but not limited to activity rooms, bathrooms, kitchen, common area, and back yard. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 72 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ restroom was measured at 113.2 degrees Fahrenheit.

All toilets, hand washing, and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for residents. Smoke and carbon monoxide combination were observed during visit. Fire extinguisher last serviced on 9/2/2025. The Fire Drill was reviewed to be last conducted on 1/20/2026. The Emergency Disaster Plan was last reviewed on 1/20/2026.
