Facility Evaluation Report
On 4/3/2026 at 2:00 PM, Licensing Program Analyst (LPA) Y. Brown arrived unannounced to conduct a Case Management visit. LPA met with Christopher Ibhawo, Administrator and explained the purpose of the visit.
While LPA Y. Brown was conducting a complaint investigation (15-AS-20251210152648) on 12/17/2025 and 1/8/2026 during observation, interview and record review LPA discovered:
2. LPA observed that there was unlocked medication in the refrigerator.
3. No incident reports were submitted to Licensing regarding C1's elopement or any other incidents happening at the facility.
*A total civil penalty of $500.00 ($500.00 for absence of supervision) will be assessed on today's date*
Deficiencies are cited per Title 22 California Code of Regulations and listed on LIC9099D. Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
