Facility Evaluation Report
On 6/11/2026 at 8:00 am, Licensing Program Analyst (LPA) Y.Brown conducted an unannounced annual required inspection. LPA met with Program Director, Christia- Marie Mendoza-Vasquez, and explained the purpose of the visit.
The facility’s fire clearance was approved for seventy (70) clients, fifty-five (55) may be ambulatory and fifteen (15) may be non-ambulatory. LPA toured the facility including but not limited to activity rooms, bathrooms, kitchen, common area, and back yard. The Day Program facility consists of five (5) rooms, including an office space, and three (3) restrooms. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 72 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ restroom was measured at 114.3 degrees Fahrenheit.
All toilets, hand washing, and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for clients. Smoke and carbon monoxide combination were observed during visit. Fire extinguisher last serviced on 06/18/2025. The Fire Drill was reviewed to be last conducted on 4/30/2026. LPA observed The Emergency Disaster Plan and it was last reviewed on 4/10/2026. The First aid kit was observed to be complete. Six (6) staff records were reviewed and six (6) client records reviewed and all are current. LPA reviewed a sample of medication.
