The inspector’s account

On 6/19/2025 at 9:25AM, Licensing Program Analysts (LPAs) G. Luk and Y. Brown arrived unannounced to conduct Case Management Inspection in regards to the incident report that was received on 6/10/2025. LPAs met with Program Director, Christia Vasquez and explained the reason for the visit.

Incident report dated 6/10/2025 states that during an outing with clients, C1 was sitting far from the group. Staff noticed that C1 was no longer present in the area and was observed to be far from the group. Staff proceeded to follow C1, but staff lost sight of C1. Staff called police for assistance and family was notified. Police located C1 and was brought back to the police station. Staff and family met at the police station and family took C1 home.

During visit, LPAs reviewed C1's files and interviewed staff. LPAs observed C1 did not have a medical assessment on file.

The deficiency was observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct deficiency may result in civil penalties.
