Facility Evaluation Report

On 10/6/2025 at 9:45 am, Licensing Program Analyst (LPA) Y. Brown conducted an unannounced Case Management visit regarding an incident that was reported to CCLD on 9/3/2025. LPA met with Program Director Christiamarie Mendoza and Administrator Gabriel Mendoza and explained the purpose of the visit.

LPA interviewed S1 and S2, obtained and reviewed C1's physician's report, IPP, ISP and PE. The Incident report for C1 was sent on 9/3/2025. The incident occurred on 9/2/2025. Incident report stated C1 had an unwitnessed fall but C1 did not have any visible injuries. The incident report stated that C1 later complained of pain on their right foot and had difficulty walking.

No deficiencies cited at this time. LPA may return at a later date.
