Facility Evaluation Report
On 6/19/2025 at 9:25am, Licensing Program Analysts (LPAs) Y.Brown and G. Luk conducted an unannounced annual required inspection. LPAs met with Program Director, Christiamarie Mendoza/Vasquez and explained the purpose of the visit.
The facility’s fire clearance was approved for seventy (70) residents, fifty-five (55) may be ambulatory and fifteen (15) may be non-ambulatory. LPAs toured the facility including but not limited to activity rooms, bathrooms, kitchen, common area, and back yard. The Day Program facility consists of five (5) rooms, including an office space, and two (2) restrooms. All indoor passageways are kept free of obstruction. There are no bodies of water. A comfortable temperature for clients is maintained at 72 degrees Fahrenheit. LPAs observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ restroom was measured at 116.5 degrees Fahrenheit.
All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Hand washing poster, paper towel, and soap observed at all hand washing stations. The supply of extra hygiene was available for residents. Smoke and carbon monoxide combination were observed during visit. Fire extinguisher last serviced on 06/18/2025. The Fire Drill was reviewed to be last conducted on 5/30/2025.
