On this day at approximately 10:40 am, Licensing Program Analyst (LPA) arrived at the facility unannounced to conduct case management visit to follow up on six residents that needed immediate transfer from Montgomery Springs Manor. LPA met with Administrator Dolly Rizvi. LPA explained to Rizvi the purpose of visit.
During the visit, LPA interviewed Residents 1-6.
Based on interviews conducted, there are no immediate health and safety concerns noted.
