On 07/24/2023 at 09:06 AM Licensing Program Analysts (LPA) J. Clancy-Czuleger arrived unannounced to conduct a Case Management. LPA met with Alanna Spencer, Administrator.
LPA discussed an incident report that was submitted on 6/3/23. The facility gave additional details for the situation stating that they had submitted a report saying that the client did return to the facility and had not used. The resident is pregnant and will soon be placed at a new facility for the remainder of the pregnancy for the health of the baby.
