Facility Evaluation Report
On 1/29/2026 at 11:30 AM , Licensing Program Analyst (LPA) Y. Brown arrived unannounced to conduct a case management visit. LPA met with Director of Nursing, Lisa Rodriguez.
During the course of investigation for complaint (#15-AS-20260123124712), the following deficiencies were observed:
1.) LPA observed unlocked medication in a residents room.
2.)LPA observed that the facility did not report an incident regarding a resident.
*An immediate civil penalty of $250 will be assessed on today's date for failure to correct and repeat violation.
The deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct deficiencies may result in civil penalties.
Administrator Della authorized Lisa to sign today's report.
Exit interview conducted with Lisa and a copy of this report and appeal rights provided.
