Facility Evaluation Report

On 4/8/2026 at 1:30 pm, Licensing Program Analyst (LPA) Y. Brown arrived unannounced to conduct a case management visit due to receiving an eviction notification for R1. LPA met with Administrator, Ryan Carvajal and explained the purpose of the visit.

R1 was admitted to the facility on 11/19/2024 and is still residing at the facility. Administrator states that R1 has been refusing medication and refusing dialysis treatments. Administrator states that they issued the eviction notice because they are not able to meet R1's needs. Administrator states that currently they are still trying to work with R1 and R1's case manager to find a feasible solution for R1 and is scheduled to have a meeting soon about R1's needs.

No deficiencies cited at this time. Exit interview conducted and a copy of this report provided.
