Facility Evaluation Report

On 1/7/2026 at 1:35 pm, Licensing Program Analyst (LPA) Y. Brown conducted an unannounced annual required inspection. LPA met with Administrator Dominique Mellion and explained the purpose of the visit. The administrator certificate # 7003760735 expires 1/10/2026. The facility’s fire clearance was approved for six (6) ambulatory clients. No clients present during visit.

LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of four (4) bedrooms and two (2) bathrooms. All indoor and outdoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature for clients is maintained at 70 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 120 degrees F. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for residents. There is a minimum of 7-day non-perishables and 2-day perishables foods.

Smoke detectors and carbon monoxide were observed during visit. Fire extinguisher was last serviced on 2/3/2025. Emergency disaster plan last reviewed on 6/24/2025. Fire drill last conducted 12/13/2025. First aid kit was observed to be complete.
