Facility Evaluation Report

On 5/6/2026 at 12:00 PM, Licensing Program Analyst (LPA) Y. Brown arrived unannounced to conduct a Case Management visit. LPA met with Azziz Ikharo, Administrator and explained the purpose of the visit.

While LPA Y. Brown was conducting a complaint investigation (#15-AS-20251106085302) on 11/07/2025 during facility tour and interview LPA observed:

1. Imodium A-d loperamide HCL oral solution 8 oz "anti-diarrhea" bottle that was not prescribed to C1 but staff was providing it to C1. .

The deficiency was observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties.
