Facility Evaluation Report

On 4/22/2026 at 11:30 AM, Licensing Program Analyst (LPA) Y. Brown arrived unannounced to do an annual 1-year required inspection. LPA met with care staff Florence Erhabor and explained the purpose of the visit. Florence phoned the administrator (ADM), Azziz Ikharo and the ADM arrived shortly after.

The administrator currently holds a certificate (# 6071064735 ) that expires on 7/24/2026 . LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage and back yard. All indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature for clients is maintained at 73 degrees Fahrenheit. The water temperature was measured at 114.8 degrees Fahrenheit.

The Emergency Disaster Plan was last reviewed on 4/22/2026. LPA observed lighting in all rooms are adequate for the comfort and safety of the clients. There is a minimum of one week supply of non-perishables and 2-day perishables food supply. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for residents. Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced on 7/15/2025. First aid kit was observed to be complete. LPA reviewed five (5) client records and five (5) staff records. LPA reviewed a sample of medication. LPA reviewed client P & I.
