Facility Evaluation Report

On 04/23/2025 at 9:30 AM, Licensing Program Analysts (LPA)s Y. Brown and L. Holmes conducted an unannounced case management visit regarding upon the incident LPA received on 03/19/2025. LPAs spoke with Administrator Azziz Ikharo and explained the purpose of the visit.

The LPAs requested the client (C1) records for review. LPAs reviewed client (C1) physician's report from 2017 & 2019 and LPAs discovered that C1's previous physicians report states that C1 is able to leave unassisted from the facility. LPAs interviewed Administrator (AD) and discussed that C1 is doing better after the incident and has not been having "outbursts" or difficulties going to C1's day program. AD mentioned that C1 had a change of medication that has helped C1 become calmer and C1 has been getting more sleep which has helped C1 with having a better mood.

AD agreeded to submit C1's updated physician's report to LPA Y. Brown via email by 05/07/2025.

No deficiencies cited. Exit Interview conducted and a copy of this report provided.
