Facility Evaluation Report

On 05/23/2025 at 2:50PM AM, Licensing Program Analysts (LPAs) Y. Brown and L. Fontanilla conducted an unannounced required annual inspection. LPAs met with caregiver Maria Teresa Tuazon. Licensee Alex Juntilla arrived at 3:05 PM and LPAs explained the purpose of the visit.

The facility’s fire clearance was approved for six (6) non-ambulatory clients. LPAs toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. The facility consists of six (6) total bedrooms and 2/6 bedrooms are used for staff and two (2) bathrooms.

There are no bodies of water observed. A comfortable temperature for clients is maintained at 73.9 degrees Fahrenheit. LPAs observed lighting in all rooms are adequate for the comfort and safety of the clients. Hot water temperature in the shared clients’ bathroom was measured at 112.7 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. Hand washing poster, paper towel, and soap observed at all hand washing stations. The supply of extra hygiene was available for residents. Smoke detectors/carbon monoxide were in operating condition during visit.
