Facility Evaluation Report

On 07/28/2026 at 12:45 pm, Licensing Program Analyst (LPA) Y. Brown conducted an unannounced annual 1-year required inspection. LPA met with Administrator (ADM) Alex Juntilla and explained the purpose of the visit.

The ADM currently holds a certificate (#7035276740) that expires on 11/17/2026. LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area, garage, and back yard. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature for residents is maintained at 72 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents.

Hot water temperature in the residents shared bathroom was measured at 116.4 degrees Fahrenheit. All toilets, hand washing, and bathing are safe, sanitary and in operating condition. The supply of extra hygiene was available for residents. Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced on 12/4/2025. First aid kit was observed to be complete. LPA reviewed four (4) staff and four (4) resident records. Emergency disaster plan was last reviewed on 9/12/2025. Emergency disaster drills were last conducted on 4/10/2025. LPA reviewed a sample of medication.
