On 4/10/2026 at 12:10 pm, Licensing Program Analyst (LPA) Y. Brown arrived unannounced to conduct a case management visit to deliver amended report originally dated 3/12/2026. LPA met with Caregiver, Norma Cabrera and informed her the reason for visit.
Staff did not have the original report dated 3/12/2026. LPA will obtain the original report at a later time from the Administrator.
No deficiencies are being cited on this date.
