On 3/12/2026 at 12:00 PM, Licensing Program Analyst (LPA) Y. Brown arrived unannounced to conduct a Case Management visit. LPA met with Alex Juntilla, Administrator and explained the purpose of the visit.
While LPA Y. Brown was conducting a complaint investigation (15-AS-20260309124645) on 3/12/2026 during file review LPA discovered:
1. S1 was not associated to the facility.
The deficiency was observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties.
Exit interview conducted with Alex and a copy of this report and appeal rights provided.
