Facility Evaluation Report
On 5/7/2025 at 4:00PM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to conduct a case management visit in regards to incident report received. LPA met with Resident Care Coordinator, Sherallyn Dones and informed her the reason for the visit.
Based on the incident report received, resident (R1) was taken to the hospital due to change in activity level. R1 was diagnosed with a fracture on left elbow. R1 was discharged from the hospital with a sling
Based on interview with S1, R1 did not have a fall. S1 stated R1 did not have any injuries prior to the hospital visit. R1 follow up with Orthopedics. R1's activity level returned to normal. Interview with R1 indicated that R1 was doing well and was watching TV during visit.
No deficiencies are being cited on this date.
