Facility Evaluation Report
On 12/31/2025 at 10:00 AM, Licensing Program Analyst (LPA) P. Manalo arrived announced to conduct a closure inspection visit. LPA met with Licensee, Marcie Hodge, and new property lease, Norman Goolsby III explained the purpose of the visit.
On December 16, 2025, Licensee sent CCLD a letter indicating the facility closure and surrender of the license effective December 1, 2025.
During the visit, LPA toured the facility including but not limited to bedrooms, bathroom, common area, and backyard area. LPA observed no clients present.
Interview with Licensee revealed that there has only been one client present at the facility since it opened and that client moved back to their home October 2024. Interview also indicated that the licensee owns the property and is leasing it to another individual.
Licensee stated the original license was discarded and Licensee does not have a copy of it anymore. LPA will send a forfeiture letter at a later time.
