The inspector’s account

On 02/12/2024 at 10:30 AM, Licensing Program Analyst (LPA) A. Gomez arrived unannounced to attempt a 1-Year Annual Required inspection. LPA returned to complete visit but upon contacting Administrator they informed LPA that they were unavailable.

On 03/08/2024 at 10:45 AM, Licensing Program Analyst (LPA) A. Gomez arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Administrator, Chad-Eric Montgomery and explained the purpose of the visit. The facility’s fire clearance was approved for 4 ambulatory.

LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 4 total bedrooms. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water. LPA observed lighting in all rooms are adequate for the comfort and safety of potential clients. All toilets, hand washing and bathing are safe, sanitary and in operating condition.

Currently the facility is not yet vendorized and has no clients. LPA advised administrator to notify CCLD once they begin the process of accepting clients so that an additional inspection can be done.

LPA confirmed that administrator currently holds a valid certificate. Certificate # 6063428735 Exp: 8/11/2024

No deficiencies cited during visit. Exit interview conducted and a copy of this report provided.
