On 10/13/23 at 3:30 p.m., Licensing Program Analyst (LPA) Greg Clark arrived unannounced to conduct a case management visit.
LPA knocked on the front door of the facility several times but nobody answered. LPA observed that the facility van was not parked in front of the facility.
LPA called the administrator (ADM) who reported that he was out of town and wasn't going to returned until later this evening. ADM also stated that the staff was out picking up residents at their day programs.
LPA will return at a later date to complete the visit.
