Facility Evaluation Report

On 5/1/2025, at 3:30pm, Licensing Program Analyst (LPA) L. Hall arrived unannounced to conduct a Case Management visit. LPA met with Anthony Garcia, Executive Director and explained the reason for the visit.

While LPA L. Hall was conducting a complaint investigation 15-AS-20250318163614 information was ascertained that on March 17, 2025, S1 inquired with the Oakland Regional Office regarding Provider Information Notice Summary 24-13 (PIN) and was given the incorrect information. LPA reviewed regulation 87464, specifically section (e), with S1, to provide information regarding residents that receive SSI after being admitted to the facility.
