This is an official report of an unannounced visit/investigation of a complaint received in our office on

08/14/2024 and conducted by Evaluator Gregory Clark

COMPLAINT CONTROL NUMBER: 15-AS-20240814093716

On 8/23/24 at 2:30 p.m., Licensing Program Analyst (LPA) Greg Clark arrived unannounced to conduct an initial 10-day complaint investigation and deliver findings in regard to the allegation above. LPA met with Anthony Garcia and explained the purpose of the visit.

During the course of the investigation LPA interviewed S1 and toured the Independent Living building. S1 stated that the facility has a contract for routine pest management with Orkin who come out to the facility weekly. The facility also has maintenance staff on duty to treat issues as they arise on a daily basis.

LPA did not observe any pests in the independent Living building.

This agency has investigated the complaint alleging the facility has pests. We have found that the complaint was unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
