Facility Evaluation Report
On 05/26/2026, at 9:15 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct this Plan of Correction (POC) visit concerning one of the citations issued for complaint 15-AS-20260325143003 that was due on 05/18/2026. Upon entry, the LPA informed the Administrator (ADM) Dr. Falemei "Mei" Paongo, PhD of the reason for the visit.
At approximately 9:45 AM, the LPA toured the facility with the ADM. During the tour, the LPA confirmed that four non-ambulatory residents had moved out of the facility as per the POC. The citation was cleared and a clearance letter was provided to the ADM.
No citations were issued during this visit.
