Facility Evaluation Report
On 9/26/2025 at 11:30 AM, Licensing Program Analyst (LPA) David Doidge arrived unannounced to conduct a health and safety check. LPA met wit Caregiver, Josefine Sucgano and explained the purpose of the visit. Administrator, Sheila Muniz was called by staff. LPA was given phone and informed Administrator of the visit.
LPA toured the facility including but not limited to bedrooms, bathrooms, common area, kitchen, and outdoor area. Water temperature in common bathroom was measured at 109 degrees Fahrenheit. There was sufficient supply of perishable and nonperishable foods. Multiple fire extinguishers were observed; last serviced on 03/31/2025.
